INDEMNITY FORM

Parent’s/Guardian’s consent are required for participant below 21 years of age on date of the enrolment.



I consent to the above applicant, who is my child/ward* participating in the above lifesaving course and accept all legal and other responsibilities connected with the course.



I hereby indemnify and agree to keep Lifeguard Singapore (UEM: 53414226E) its management, and organizers’ of the event fully indemnified against all claims, loss or damage whatsoever in respect of my child’s/ward’s loss of life, injury, disability or any loss or damage whatsoever arising from any cause in connection with the course or his/her participation therein.

TERM & CONDITIONS:

I, student/parent of the child/guardian of the child*, agreed to the following terms upon payment, (hereinafter called “student“) of one part

AND

Lifeguard Singapore, (hereinafter called “instructor“) of the other part”

1. Bi-monthly payment is to be made in the form of cash or Paynow before the first lesson start. Following fees, from the 3rd month onward, will be collect on a Bi-monthly basis.

2. Payment can be made via Cash, Paynow or Paylah!

3. Make-up classes will be provided solely, for students who are absent due to MC. A replacement lesson will be given on a specific date and time which the Instructor is available to teach and must be attended by the student within the next 30 days.

4. If the student wishes to take a month’s break due to personal reason(s), please note that half a month’s fees is payable due to the consideration of the swimming instructor reserving a place in the course for the student.

5. In event of any unforeseen circumstances or bad weather conditions, alternative lessons such as, and not limited to, land-drills exercise, water safety theory or Q&A session may be conducted in place of the usual practical lessons if the weather/restriction rendered the pool, unsuitable to continue any pool-based lessons safely.

6. No lesson(s) will be conducted on public holiday(s), Eve of Chinese New Year and 1st to 7th day of Chinese New Year.

7. Lesson(s) shall only be conducted for four (4) of the five (5) week(s) in any months that have a fifth (5) week of the lesson day.

8. In situation where the instructor cancels a lesson, a replacement lesson will be provided for the student at an earlier or later date and time at which the instructor is available to provide the replacement lesson. Additionally, the replacement lesson would be provided at the instructor’s sole discretion.

9. Any payment made before any force majeure event, will be received and lessons will be rescheduled at a later date in accordance to (8), if the instructor is unable to return to duty, a replacement instructor will be arranged to take over.

10. ONE (1) MONTH’S NOTICE to the instructor is required if the student wishes to stop their swimming lessons. Upon fulfilling the conditions of termination, each party shall not hold either party responsible for further obligations or services.
